Membership Conditions:

• Studio policies apply

• Treatment frequency is 1 treatment every 6 weeks

• Treatments must be used within the designated period

• Treatments cannot be carried over

• Treatments must be redeemed by the holder of the membership and can not be transferred to someone else

• Cancellation of your membership is available with 30 days written notice

• 50% of the remaining balance is required to be paid upon cancellation
